How Can We Help?

We believe that an excellent home health care team can't simply "leap" into a home care situation. Proper care begins with dialog and thought. So before we begin any assignment, we ask questions and we plan.

Our preliminary process revolves around four words: Listen, Plan, Educate, Serve.

We listen to our clients and their families to learn about situations, attitudes, personal preferences, and favorite things. We plan, with family involvement, for personal and individualized care for each client in each situation. We educate our clients, their families, and ourselves about medical history, risks and preferences. Finally we serve our clients and their families with proficiency and compassion.

Our registered nurse completes an assessment and writes an individualized nursing plan of care. Services are rendered by our Home Health Aides, Certified Nursing Assistants, and Rehabilitation Techs specific to your plan of care under the supervision of our registered nurse.
